Brian is a brain-like monster that made his first and exclusive debut in the Nintendo Wii version of Rampage: Total Destruction. He is one of 30 playable monsters (40 on the Nintendo Wii version). This is the only Rampage game he has appeared in.

- Brian's name is a reference to the fact that the word "Brian" is an anagram of "Brain".
- Brian, Sarah, Fifi, Plucky, Squirmy, Fabio, Boris, Ruby, Cyril, and V.E.R.N. are the only monsters in Rampage: Total Destruction to not be given descriptions on the official website.
- Brian could be a tribute to Brian Colin, the original creator of the Rampage series. The developers of Rampage: Total Destruction could consider Brian Colin to be a "genius" for creating Rampage, a substantial reference made by Brian the Brain's large brain.
- Brian has areas on his shoulders and upper arms that resemble his brain.
- Brian's altered physiology may be extraterrestrial in origin. The presence of Eyegore illustrates that Humans can mutate into aliens, though the how's and why's have never been elaborated upon.
- Brian makes a striking resemblance to the Martians from the Mars Attacks! franchise. This reference could also be made clear when he "invades" cities.
- In addition, Mitch the Secret Service Agent from Mars Attacks! is acted by Brian Haley. This could merely be coincidence though.
